Important Forms/Documents

• The following list of names are the students who have turned in their 2008-09 Athletic Department Physical/Consent Forms. If your name is not listed or there is a “No” in one of the columns you have not turned in your form or portion of form. If you should have any questions please contact Mr. Tim Moscicki ATC at 213-381-5121 x126 or emai. Blank forms are available at his office or can be downloaded from the website. No student will be allowed to tryout or practice until the forms have been turned into Mr. Moscicki.
